Registration is easy! Simply download the Registration Form,
and fax the completed form to 330-523-5271.
Once it is processed, an official Registration Packet will be sent to the participant(s) with detailed course information, including recommended hotel and travel information.
The Center for Exceptional Practices recognizes that times may arise when a dentist may not be able to participate in a course due to unforeseen situations. Tuition deposits may be transferrable to a future course and/or substitutes may be permitted with prior approval from The Center for Exceptional Practices.
